The thermostat and a metal extension tube that runs across the top of the engine and under the intake manifold. This was provided as one assembly or as two parts consisting of the thermostat itself and the extension tube.

My 2019 Acadia got the P0128 code (~89,000 mi). Looked at several parts stores before finding one in stock for about $170 (thermostat and housing assy). Used the following video intructions and did the job easily in about 3-4 hours. That time can easily be cut down for someone who has done it before.
