The following are checked (anything not listed is unchecked): add extra roll buttons to sheet, make item image auto-roll, alt-click image for secondary roll, show DAE active effects button, show d20 die icon. Enable apply damage buttons is disabled. Hide save DCs is set to NPCs only. These are my midi settings.

First I would install the Convenient Effects module. Your spells have to be configured for DAE to trigger reactions with MidiQoL which is what CE does for a dozen or so very common spells and class features. Midi can reference CE when triggering certain reaction spells and Shield is probably the most common type of that at early levels.I want to have Midi QOL working, but don't want it to roll, as Ready Set Rolls has the way better roll card and options. Your midi-qol settings work in my environment. I'm using an adult red dragon attacking the Zanna PC from the compendium in the 5e system and it works. You should double check and update DAE, socketlib, and libWrapper. If that doesn't work, I suggest maybe backup and deleting settings.db in your world folder.
