Make sure you have Eager Edge perk on the sword and Shatterdive equipped. Start with any other weapon (because the perk works for the 1st attack after you swap to sword), swap to the sword, use heavy attack, press and hold jump+shatter button together but with jump being a frame (5-20ms) earlier.
